Our Social Prescribing service supports patients referred by GP practices across Central Islington. Working within a fast-paced, person-centred environment, Social Prescribing Link Workers support individuals to identify what matters most to them and connect with community services, activities and support that improves their well-being and addresses the wider determinants of health. The service benefits from close collaboration with our Connect service and access to over 120 referral partners, offering a diverse range of opportunities to support residents’ health, wellbeing and independence.
Duties
- Work with patients referred by GP practices to provide personalised, strengths-based support
- Build trusted relationships and support individuals to overcome social, practical and emotional barriers
- Support people to access community-based services, activities and networks
- Manage a varied caseload, maintaining accurate and timely records
- Work collaboratively with GP practices, primary care teams, colleagues and external partners
Requirements
- Passionate about creating a fairer society and reducing health inequalities
- Excellent interpersonal and communication skills
- Experience of providing person-centred or personalised support (paid or unpaid)
- Able to work at pace and across different data systems while maintaining accurate records
- Can manage a caseload in an organised and methodical way
- Comfortable balancing face-to-face work with administrative responsibilities
- Able to provide motivational coaching to support people’s behaviour change
- NVQ Level 3, Advanced level or equivalent qualifications or working towards
- Training in motivational coaching and interviewing or equivalent experience
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity
- Experience of working with the VCSE sector (in a paid or unpaid capacity), including with volunteers and small community groups
- Experience of data collection and using tools to measure the impact of services
- Knowledge of community development approaches
