We are looking for a motivated and experienced Social Worker to join our Learning Disabilities Partnership (ILDP), delivering high-quality, person-centred services to adults with learning disabilities and their families. This is an exciting opportunity to work within a multi-disciplinary health and social care team, supporting individuals to live independent, fulfilling lives while ensuring their safety, wellbeing, and rights are upheld.
What You’ll Be Doing
- Undertake strengths-based assessments, care planning, and reviews tailored to individual needs.
- Work collaboratively with service users and carers to achieve person-centred outcomes and promote independence.
- Identify and manage risk and safeguarding concerns, ensuring timely and appropriate intervention.
- Operate within a multi-agency environment, working closely with health partners, housing, and community services.
- Apply relevant legislation, including the Care Act and Mental Capacity Act, in all areas of practice.
- Maintain accurate and high-quality case records in line with organisational standards.
- Support individuals to explore a range of care options, including alternatives to council-funded services.
- Contribute to service improvement and performance within a dynamic and supportive team.
What We’re Looking For
- Are qualified Social Workers and registered with Social Work England.
- Have experience working with adults with learning disabilities.
- Demonstrate strong knowledge of safeguarding, risk assessment, and statutory responsibilities.
- Are committed to person-centred, strengths-based practice.
- Have excellent communication and partnership working skills.
- Are motivated, resilient, and able to work effectively in a fast-paced environment.
