Responsibilities:
- Liaise with professionals and agencies; ensure safe and effective daily operations, handle clinical inquiries.
- Communicate complex, sensitive information clearly to individuals and families; participate in clinical meetings, including CPA reviews and risk assessments.
- Employ empathic communication and negotiation skills; promote social inclusion and fair access to social care services.
