Devon Partnership NHS Trust is seeking an experienced and forward-thinking Senior Clinical Pharmacist to join our expanding Medicines Optimisation service. The post holder will provide a specialist clinical pharmacy service to mental health community rehabilitation teams across the Trust, supporting safe, effective and person‑centred use of medicines for people with complex mental health needs.
Responsibilities
- Provide specialist clinical pharmacy input to community rehabilitation teams, including medication reviews, therapeutic monitoring, and complex medicines consultations.
- Support safe prescribing and medicines optimisation across rehabilitation pathways, working closely with medical and non‑medical prescribers.
- Act as a key contact for people who use our services, their families and carers, delivering clear medicines‑related information and promoting shared decision‑making.
- Work collaboratively within multidisciplinary community and inpatient teams, contributing to clinical meetings, care planning and risk management.
- Liaise with PCN GPs, primary care pharmacy staff and wider primary care colleagues to ensure seamless transitions of care and accurate medicines reconciliation.
- Provide specialist advice, guidance and training to DPT staff, supporting governance, audit, policy development and quality improvement initiatives.
- Engage in professional networks and supervision groups with specialist mental health and learning disability pharmacists to maintain and develop advanced clinical skills.

Qualifications
Essential:
- Current registration as a Pharmacist with the General Pharmacy Council (GPhC).
- Evidence of Continuing Professional Development.
- Postgraduate diploma in clinical pharmacy / therapeutics or equivalent clinical experience.
- Certificate/Diploma or Masters in Psychiatric Pharmacy practice or equivalent clinical experience.
- Independent Prescribing Qualification (or be working towards completion).
Desirable:
- Member of the College of Mental Health Pharmacy.
Experience
Essential:
- Experience of working within a multidisciplinary healthcare team / environment.
- Ability to supervise activities of pharmacy technicians and assistants.
- Managing change in a complex environment and evidence of having successfully implemented change.
- The ability to work under pressure, to deadlines where there are conflicting priorities.
