Community Mental Health Practitioner / Nurse

The OPMH East Devon Community Team, based at St Johns Road in Exmouth, is seeking a committed and compassionate Community Mental Health Practitioner / Nurse to deliver high quality mental health services across the East Devon area.
About The Role: As a key member of our multi-disciplinary team, you will manage a defined caseload, providing continuous assessment and personalised treatment to individuals with a range of mental health presentations. This rewarding role is central to supporting peoples recovery and wellbeing.
- Key working: Design and co-ordinate bespoke treatment packages tailored to individual needs.
- Caseload management: Take responsibility for assessment, planning, implementation, and evaluation of care.
- Liaison and partnership: Work closely with statutory and non-statutory agencies to ensure holistic support for service users and their carers.
- Team collaboration: Contribute effectively as an integral part of our multi-disciplinary team, sharing expertise and supporting joint working.
Main duties of the job:
- Contribute to the teams engagement including liaising with a wide range of other professionals and agencies.
- Impart complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment, and review.
- Multi-disciplinary teams to ensure the provision of consistent well co-ordinated care.
- Primary care teams regarding client care.
- External agencies also involved in the provision of care e.g.: voluntary and private sector, police, social services, housing, employment etc.
- Service user/carer groups.
- Contribute to the safe and effective day to day operations of the service including the handling of clinical enquires.
- Provide and receive information some of which may be contentious, sensitive and complex to and from individuals and their families/carers and actively seek support as appropriate.
- Provide a high standard of written and verbal communication that is clear and in line with professional documentation standards.
- Participate in clinical meetings, reviews, professional meetings, and clinical risk meetings.
- Communicate information in a way that makes it relevant and understandable for service users and carers, working in line with practice standards and operational policies.
- Facilitate fair access to social care services and promote social inclusion regardless of age, ethnicity, gender, sexual orientation or disability.
- Actively contribute to multi-disciplinary team working.

- An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
- An understanding of CPA Process.

- Understanding of recovery principles in Mental Health.
- Understanding of Health and Social Care Agenda for Older Adults.
- Understanding of Organic Presentations in Older Adults.
- Managing a caseload of clients with a range of mental health presentation.
- Experience of working with older adults with mental health problems.
- Working within a community setting in either the statutory or voluntary sector.
- Providing clinical supervision.
- Experience of working in a multi-disciplinary and multi-agency environment.
Essential Qualifications:
- An appropriate professional Mental Health qualification and registration e.g. RMN, OT or Social Worker.
Desirable Qualifications:
- Training to level 2 in Safeguarding Adults, children and young people: roles and competencies for Health Care staff.
