As a Mountbatten Healthcare Assistant, you will deliver high quality Specialist Palliative and End of Life care as part of the Inpatient Unit multidisciplinary team, under the direction of the designated nursing team.

You will be required to work unsocial hours as required, including weekends/nights and bank holidays.

Overview of Role

Supporting the assessment, planning and evaluation of individualised patient centred care, ensuring a holistic approach that is sensitive and responsive to the patients’ ever-changing needs. Maintaining the patient’s comfort, dignity and privacy at all times.

Ensuring all aspects of the care provided are provided in accordance with Mountbatten’s Values and Care Standards.

Key Tasks

· To deliver high standard of direct patient care under the supervision and direction of qualified employees, ensuring all changes in a patient’s condition are reported immediately to the Senior Nurse on duty/Sister or Deputy Sister/Clinician

· Ensure timely and accurate recording of care provided and patient information on SystmOne, in accordance with Mountbatten’s procedures

· Working as part of the multi-disciplinary team contribute to discussions and patient care as required, acting as an advocate for the patient’s wishes as required

· Accompany patients to appointments outside of Mountbatten, as requested by the Sister, Deputy Sister or nurse in charge

· To ensure that patients and families spiritual, psychological and social needs are meet in accordance with their wishes
