About the role
These are limited-term appointments, commencing September 2026 and ending August 2027. As a Registered Nurse within our District Nursing Team, you’ll deliver skilled, evidence‑based care to patients with complex needs in their homes, residential and other community settings. While working independently and alongside a multi‑disciplinary team, you’ll assess, plan and implement personalized care that supports independence and helps prevent avoidable hospital admissions.
