Er mwyn ein galluogi ni i wella ein hymateb i’r ddyletswydd hon; rydym yn sefy dlu tîm newydd a fydd yn darparu teuluoedd â chefnogaeth ddwys a chyflym gyda’r nod o gryfhau gwydnwch, atal teuluoedd rhag chwalu a hyrwyddo newid fel bod yr amgylchedd teuluol yn un diogel.

Nod y Tîm Teuluoedd Gwydn fydd:

  • Gweithio’n ddwys â theuluoedd er mwyn cadw eu plant allan o ofal.
  • Gweithio’n ddwys â theuluoedd er mwyn ailuno plentyn â’i deulu o fewn 8 wythnos o’r adeg pryd y dechreuodd dderbyn gofal.
  • Gweithio’n ddwys â theuluoedd sydd wedi’u hadnabod er mwyn eu hailuno gyda’u plant sydd mewn gofal ar hyn o bryd ac achos lle mae dychwelyd adref wedi cael ei nodi fel cynllun parhaol ar eu cyfer yn y tymor hir.

Bydd y Tîm yn cynnwys gweithwyr proffesiynol sydd â’r sgiliau a’r profiad o weithio’n uniongyrchol â phlant sy’n derbyn gofal a chefnogaeth, eu rhieni ac oedolion ag anghenion iechyd a gofal cymdeithasol cymhleth. Bydd y tîm yn gweithio’n rhagweithiol ac yn ddwys gyda theuluoedd er mwyn rheoli risg, ymateb yn ddigyffro i argyfyn g a thrwy ffyrdd arloesol, cymorth ymarferol a sgiliau therapiwtig, cryfhau gwytnwch, gallu rhianta a lleihau risg o fewn teuluoedd, er mwyn sicrhau gwell canlyniadau ar gyfer plant a phobl ifanc.
