We are now seeking an experienced care coordinator to join our team, a pivotal leadership role providing clinical oversight and care governance across our supported living services. This role is ideal for a registered nurse with a strong background in community-based nursing or complex adult care who thrives in a fast-paced evolving environment.

Main duties of the job:

  • Provide clinical leadership across supported living services.
  • Review and implement care plans for individuals with complex or high-risk needs.
  • Act as the lead clinical liaison for referrals, transitions, and MDT meetings.
  • Support service managers to embed best practice and maintain CQC compliance.
  • Monitor incident reports, safeguarding alerts, and health outcomes.
  • Lead on health audits, clinical risk assessments, and medication reviews.
  • Train and mentor support staff in trauma-informed high-quality care.
  • Work collaboratively with local authorities, NHS commissioners, and families.

Person Specification:

Experience:

  • Essential: Minimum 3 years post-registration experience in a clinical or Care Coordination role; experience in supported living, residential care, or community adult health.
  • Desirable: Strong understanding of CQC, DoLS, MCA, and risk management frameworks; excellent communication, leadership, and organisational skills; ability to travel between services (mileage reimbursed); experience with forensic clients or dual diagnosis; experience supporting services through CQC inspections.

Qualifications:

  • Essential: NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
