As Care Coordinator, you will play a central role in ensuring safe, high-quality, person-centred care across our services. You will work closely with operational and quality teams, offering clinical leadership and supporting best practice in line with CQC standards.
Key Responsibilities
- Provide clinical leadership across supported living services.
- Review and implement care plans for individuals with complex or high-risk needs.
- Act as the lead clinical liaison for referrals, transitions, and MDT meetings.
- Support service managers to embed best practice and maintain CQC compliance.
- Monitor incident reports, safeguarding alerts, and health outcomes.
- Lead on health audits, clinical risk assessments, and medication reviews.
- Train and mentor support staff in trauma-informed, high-quality care.
- Work collaboratively with local authorities, NHS commissioners, and families.
Essential Requirements
- NMC Registered Nurse (RMN, RNLD, or RGN with community/complex needs experience).
- Minimum 3 years' post-registration experience in a clinical or care-coordination role.
- Experience in supported living, residential care, or community adult health.
- Strong understanding of CQC, DoLS, MCA, and risk management frameworks.
- Excellent communication, leadership, and organisational skills.
- Ability to travel between services (mileage reimbursed).
Highly Desirable
- Experience with forensic clients or dual diagnosis.
- Experience supporting services through CQC inspections.
