We are seeking a Senior Staff Nurse/Paramedic to join our Hospice at Home Team to provide individualised holistic care to patients across eastern Hertfordshire. This role will work closely with the Isabel Hospice Community Specialist Team and In Patient Unit and includes weekend working. You will work with other health/social care professionals in the community, hospitals, and Acute Trust to ensure that patients requiring end of life support receive care in an integrated, timely manner and are referred to other parts of the Isabel Hospice service as required.
These patients will have a palliative diagnosis such as dementia, cancer, organ failure and neurological conditions and will have been referred to our services by fellow health care professionals, the patient or family.
Qualifications
To apply you should be a Level 1 registered Nurse or registered Paramedic with a minimum of two years post registration experience, preferably within the community. Experience of palliative/end of life care would be desirable.
