We are looking for an experienced Heart Failure Clinical Specialist (Nurse/Paramedic) to join our community team. This is an exciting opportunity to play a key role in delivering high-quality specialist heart failure and palliative/end-of-life care for patients.
You will work across eastern Hertfordshire in partnership with acute hospital trusts, community teams, and primary care services to ensure patients and those important to them receive seamless, compassionate, and timely care. You will also work closely with the Community Specialist team, the Hospice at Home team and In Patient Unit and may on occasion provide cover to these services if required.
Responsibilities
- Collaborate with acute hospital trusts, community teams, and primary care services to deliver seamless, compassionate, and timely heart failure and palliative/end-of-life care.
- Work across the Community Specialist team, Hospice at Home team, and In Patient Unit; provide cover as required.
- Autonomously manage a caseload with support from the multidisciplinary team.
Qualifications
- Level 1 registered Nurse or registered Paramedic, with a minimum of three years post-registration experience, preferably with community experience.
- Heart failure experience and palliative care or oncology experience are essential.
- A palliative care or oncology qualification and/or a non-medical prescribing qualification would be desirable.
