Ireland Wood & Horsforth Medical Practice is a thriving GP practice in Leeds, serving a second‑largest patient population in the city of approximately 28,000 patients across two sites.
We are seeking an experienced, motivated, and forward‑thinking Nurse Manager to lead and mentor our dedicated nursing team, ensuring the highest standards of clinical care, safety, and professional development.
Responsibilities
- Provide line management to the nursing team, including annual appraisals, 1:1s, team meetings, managing annual leave and absence, and opportunities for training and development.
- Coordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs.
- Act as the Infection Prevention Control (IPC) lead, working to the IPC policy to provide audits, training, support and evidence of compliance to the practice manager.
- Work alongside the practice manager to maximise practice income and achievement (e.g., QOF, IIF, LES/DES/Public Health targets).
- Develop, implement and embed an effective care management programme for frail and housebound patients registered at the practice.
- Assess the needs of patients, ensuring the provision of healthcare is appropriate, incorporating evidence‑based practice.
- Develop, implement and embed health promotion and wellbeing programmes.
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
- Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to patients' health.
- Provide routine nursing care to patients in accordance with clinical‑based evidence, NICE and the NSF (as required).
- Diagnose and manage acute and chronic conditions (as required).
- Prescribe medication for patients in accordance with PGDs and own scope of practice.
- Review patient medications to enhance compliance.
- Provide guidance to patients on the use of prescribed and over‑the‑counter medications regarding side effects and dosages.
- Co‑ordinate the provision of travel medicine services.
- Request pathology services and process pathology results as required.
- Oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
- Maintain accurate clinical records in conjunction with extant legislation.
- Ensure SNOMED CT codes are used effectively by all members of the nursing team.
- Prioritise health issues and intervene appropriately.
- Recognise, assess and refer patients presenting with mental health needs.
- Implement vaccination programmes for adults and children.
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
- Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
- Support the clinical team with all safeguarding matters, in accordance with local and national policies.
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
Qualifications
- A current GP Practice Nurse with experience.
