Specialist Heart Failure Nurse

Are you a clinically confident heart failure specialist nurse making a real impact from the comfort of your home base, supporting patients, carers, and multidisciplinary teams across the NHS.

The Role

As a Specialist Heart Failure Nurse, you will act as the clinical anchor providing expert guidance, education, and support to your patients. Working remotely, you will deliver high-quality clinical engagement through virtual platforms, and flexible scheduling that fits a 3-days-per-week working pattern. You will play a vital role in raising standards of care, strengthening clinical understanding, and supporting best practice across UK heart failure services.

What You will Do

  • Deliver specialist virtual support to prescribers, patients and caregivers on therapy use and disease understanding.
  • Work fully in line with Professional Body standards, NMC Code, safeguarding frameworks and confidentiality requirements.
  • Provide targeted patient training where appropriate.
  • Act as programme and project champion, contributing to cross-functional initiatives.
  • Manage and respond to medical/clinical queries from HCPs promptly and professionally.
  • Complete mandatory Adverse Event / Product Complaint training and ensure timely reporting in line with UK requirements.

What You Bring

  • RGN – Registered General Nurse (NMC-registered).
  • Minimum 5 years post-registration experience in healthcare, pharma, cardiology, or related fields.
  • Strong clinical competence in Heart Failure, cardiology, or chronic disease management.
  • Experience educating MDTs and supporting clinical decision-making.
  • A solid understanding of NHS pathways, community–hospital interfaces, cardiac rehab services, and heart failure patient journeys.
  • Excellent planning, organisation and remote self-management.
  • Confident relationship-builder across clinical and non-clinical stakeholders.
  • Comfortable working independently while contributing to a wider programme team.
  • Tech-confident: able to run virtual sessions, document electronically, and manage remote communications smoothly.
