Social Work Assistant – Adults – Hackney

iPeople SC Solutions is recruiting a Social Work Assistant to work for our client based in Hackney and surrounding areas.

Responsibilities include:

1. Providing support, information, advice, and technical assistance to individuals.
2. Developing, costing, negotiating, and mediating individual support plans as required.
3. Enabling individuals to take control of their support plans and resources.
4. Assessing people who are eligible for social care support to develop their own solutions and enhance their quality of life through promoting choice.
5. Working jointly and alone to develop support plans with people and implement these to enable them to be healthy, safe, and well.
