iPeople SC Solutions is recruiting for Older Adults Social Workers to work for our client based in Exeter, Devon. This South West of England council are looking for Social Workers who have experience working in Adult social care.
Working with vulnerable adults with long term physical and mental health needs who are at risk and in need in the community. The work will include:
- Care Act Assessments, reviews and commissioning care.
- Completion of Mental Capacity Act assessments and Best Interest decisions alongside safeguarding work.
- Hospital discharge work or support for people to return home from 24 hour care following hospital discharge.
This work is required to support teams with an increase in workload, to reduce wait times and respond in a timely way to mitigate risks. You would have a team to work alongside for support.
The worker will need to be able to demonstrate COVID safe working awareness; support and guidance will be provided. Most work could be completed remotely, i.e. working from home, but some face-to-face visits could be required including visits to care homes to complete assessments and reviews. Knowledge of the care first system is an advantage but not essential.
