Chester and Cheshire West – Adults Services
The Home Assessment team are looking for dynamic Social Workers to support in reviewing people discharged from hospital with care into the community to ensure that their independence is maximised.
- Completion of strength-based assessment, mental capacity and best interest where appropriate.
- Safeguarding investigations, care planning and case management of those discharged from hospital.
- Use of own vehicle to carry out visits in the community.
2 years PQE. SWE Registered.
