iPeople SC Solutions is currently recruiting for Social Workers (Adult services) to work for our client based in the Southwark and surrounding area. The successful post holder will work in an integrated Health and Social Care Team that covers short-term rehabilitation and Reablement for service users recently discharged from hospitals, rehab units and onto community therapy services specialising in stroke and neurological rehabilitation.

Duties and responsibilities as an Adult Social Worker:

  • Carry out home visits
  • Completing Care Act assessments
  • Reviews of long-term care provision
  • Mental capacity assessments relating to care support, change in accommodation and finances

Requirements of the Adult Social Worker role:

  • Must have minimum 4 years post qualification social work experience required
  • Enhanced DBS Check on the update service
  • Experience of managing a complex case load
  • Experience and skills preferred in areas of rehabilitation settings, dementia, neuro, stroke and Parkinson services, hospital settings and familiarity working in multidisciplinary teams and with older people and adults with physical disabilities
  • Experience of producing reports for a variety of audiences
  • Good IT skills and willingness to learn new systems
  • Able to work independently and within a team
  • Must also have knowledge of methods to plan and review services for the client group with focus on Reablement and strength-based approaches
  • Strong influencing and negotiating skills with clients and other health professionals

Working hours: 09.00- 17.00, Monday - Friday
