Salary: Up to Β£28,049 per annum (pro rata, and depending on experience)
Contract: Fixed-Term Contract until 31 March 2027

Based: Involve Head office, Medway Peninsula PCN sites, and in GP practices: Highparks Medical Practice and The Elms Medical Practice.
We are looking for a Complex Care and Frailty Social Prescribing Link Worker to work within Medway Peninsula PCN, supporting people with frailty, complex health conditions and wider social care needs. Taking a holistic, person-centred approach, you will work alongside individuals to improve their wellbeing, independence and quality of life. You will support the delivery of the new SNH Complex Care Model, contribute to Comprehensive Geriatric Assessments (CGA), utilise the Kent and Medway Care Record (KMCR), and connect people to community services and practical support that meet their individual needs.
You will need to be able to drive and have access to a vehicle for this role, as it involves community and home visits. The role includes providing regulated activity with adults, so employment is subject to an Enhanced DBS check.
