Waypoint has expanded to the Western Region of Maine, formerly known as The Progress Center. Our department is growing! Enjoy a collaborative team-based approach with small caseloads? Waypoint is seeking a skilled, motivated, and highly organized professional to join our growing Adult Case Management team to provide services throughout the Western Maine area.

Case Managers provide support to adults with intellectual disabilities/autism and their families; serving as an advocate, ensuring choice and satisfaction with services, and monitoring the individual's health and safety. Our home office for Adult Case Management is located in our Oxford, Maine location; this is a hybrid remote position after successful onboarding.

About Waypoint: Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.

About the Role: The Case Manager assists the individual and their team to identify needs and develop a plan of care, initiate referrals and monitor services, while working closely with the multi-disciplinary team. The Case Manager coordinates and advocates for services identified in the plan, monitors progress and evaluates the effectiveness of services.

Qualifications:

  • BA/BS in human services or closely related field or BA/BS in unrelated field + minimum 1 year experience in adult IDD services
  • Knowledge of the system of care in Maine preferred

Requirements:

  • Reliable vehicle with proper insurance
  • Valid driver’s license issued by state of residence with clean driving record
  • Must be proficient in speaking, reading, and writing English with strong interpersonal and communication skills.
  • Must be able to complete agency and state mandated training programs with proficiency.

Job Type: Full-time

Pay: From $24.00 per hour
