  • Demonstrate respect, attention, and awareness toward the diversity among the people we serve (residents, families, caregivers, etc.) through their attitude, service, and actions.
  • Be knowledgeable of the individualized care plan for residents and provide support to the resident in accordance with their care plan.
  • Assist in daily requirements and tasks in care and treatment of residents, including bathroom assistance, general personal hygiene (bathing, dental hygiene, hair and nails), dressing, mobility assistance, food intake, and other needs in keeping with the individuals’ care requirements.
  • Maintain resident rights and confidentiality.
  • Document all ADLs in the electronic health record.
  • Observe and report symptoms, reactions, and other changes in condition promptly.
  • Routinely turn and position residents as required by the care plan.
  • Comply with all aspects of residents’ rights and promptly report any incidents or evidence of resident abuse or violations of resident rights to the charge nurse or administration.
  • Honor residents' right to refuse care and report such refusal to the nurse supervisor.
  • Assist in maintaining a safe, neat, and clean environment.
  • Answer resident call bells promptly and courteously.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Take and document temperature, pulse, respiration rates, food and fluid intake and output, and heights and weights, and record in EHR.
  • Ambulate and transfer residents using the appropriate assistive devices and body mechanics.
  • Use only the equipment you have been trained to use, operate in a safe manner, and report any defective equipment to the supervisor immediately.
  • Communicate and interact effectively and tactfully with the residents, visitors, families, peers, and supervisors.
