Lead with Compassion, Care with Purpose. We are looking for dedicated and experienced Team Leader to join our team! At Holme Lodge we specialise in supporting people who have varying needs as a result of physical and long-term neurological conditions, including cerebral palsy, epilepsy, head/brain injury, motor neurone disease, multiple sclerosis, muscular dystrophy, Parkinson’s disease, speech impairment, spina bifida and hydrocephalus.
What You’ll Be Doing:
- Lead and mentor a team of support workers to deliver exceptional care
- Develop, implement and review person-centred care plans
- Provide practical and emotional support with daily living, personal care, and activities
- Monitor outcomes and document changes in care or wellbeing
- Liaise with families, professionals, and colleagues to ensure consistent, tailored support
- Ensure compliance with safeguarding, policies, and CQC standards
- Support the onboarding and training of junior staff
What You’ll Need:
- Previous experience in a senior support working role
- NVQ Level 2 in Health & Social Care
- Must be Medication Administration trained
- Strong supervision and communication skills
- Excellent care planning & record keeping experience
- Flexibility to work various shifts including weekends and nights if required
