We are seeking to appoint a Patient Coordinator to work 20 hours a week between 8:00 AM – 6:30 PM with our friendly, patient-focused team at Ystradgynlais Group Practice for a temporary 12-month contract.
We are looking for enthusiastic, reliable, highly motivated, and friendly individuals to join our team. The successful candidates would ideally have experience working in General Practice.
Main duties of the job
The successful candidate will be a key member of the patient coordinator team. You will play a vital role in dealing with patients, both face-to-face and over the telephone. You will provide support to the GPs and clinical team by signposting patients to the appropriate healthcare professional or service. The patient coordinator will also be expected to undertake reception duties and support the administrative team. You will be required to work across our three sites.
About us
We operate across three sites: Pengorof Surgery, Ystalyfera Surgery, and Abercrave Surgery. Our team includes 9 Partners, 3 Registrars, supported by 3 Advanced Care Planning Nurses, 3 Practice Nurses, 3 Health Care Assistants, 2 Practice Pharmacists, 2 Phlebotomists, 1 Cluster Pharmacist, 2 Cluster Pharmacy Technicians, and 4 Pharmacy dispensers. We also have a Practice Manager, 2 Deputy Managers, and a large team of administrative/clerical staff.
Our practice aims to improve the health of those living in and around the Ystradgynlais area. We work closely with many local community services, including the community hospital where our GPs provide cover to the Minor Injuries Unit and collaborate with ward Consultants.
Job responsibilities
To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the multidisciplinary team. The care navigator will also be expected to undertake reception duties, supporting the administrative team in delivering a polite and professional service to the patient population.
