We are looking for an Advanced Practitioner to join our Hastings Primary Care Hub, where you will lead on delivering high-quality care to unregistered patients and work as part of a dynamic, multidisciplinary team. The role involves working closely with the Seaview Project to support individuals experiencing homelessness, those in temporary accommodation, and hostel residents, addressing complex issues such as mental ill health, substance misuse, learning difficulties, and other health-related concerns.
What You’ll Do:
- Provide assessment, diagnosis, and treatment at first contact for patients with minor illnesses.
- Prescribe medication as an Independent Prescriber following clinical examination.
- Navigate patients to alternative care settings or treat and discharge as appropriate.
- Work within IC24’s clinical governance framework to ensure the highest standards of care.
- Engage in audits, service development, and continuous improvement initiatives.
What We’re Looking For:
- Registered Nurse (NMC) with a Nursing Degree (or working towards).
- Independent Prescriber qualification.
- Physical assessment and Minor Illness course.
- Experience in General Practice or Urgent Care (minimum 2 years).
- Strong clinical decision-making skills and ability to work autonomously.
- Passion for patient care and continuous professional development.
