IC24 is seeking an experienced Advanced Clinical Practitioner to join our Primary Care team and make a real impact in the heart of the community. This is a senior clinical role where you'll work autonomously, using advanced decision-making skills to deliver safe, effective, and patient-centred care across urgent and primary care settings.

Every day, you'll help patients in the community, whether through telephone triage, remote consultations, or face-to-face assessments providing timely interventions that make a visible difference to their health and wellbeing. Most importantly, you'll be motivated by the opportunity to deliver exceptional care and shape the future of integrated primary care services.

  • Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.
  • Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
  • Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
  • Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
  • Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
  • Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.

  • Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
