  • Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.
  • Prescribe and manage medications as an independent prescriber, ensuring safe and effective treatment plans.
  • Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
  • Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
  • Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
  • Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
  • Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
  • Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.

What We Expect:

  • Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
  • Advanced assessment and minor illness qualifications (Level 6 or 7).
  • Independent prescribing qualification (Essential).
  • Proven experience in autonomous practice and clinical leadership.
  • Strong communication skills, able to manage complex and sensitive situations.
  • Ability to prioritise, problem-solve, and make decisions under pressure.
  • Commitment to continuous professional development and reflective practice.

Desirable:

  • Experience in chronic disease management.
  • Teaching and assessing qualification.
  • Remote consultation experience.
  • Evidence of service improvement and innovation.
