This is a senior clinical role where you'll work autonomously, using advanced decision-making skills to deliver safe, effective, and patient-centred care across urgent and primary care settings. You'll be part of a highly skilled, supportive team committed to improving access to care and reducing pressure on hospitals. - Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.

- Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
- Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
- Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
- Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
- Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
- Work flexibly, including evenings, weekends, and bank holidays, and travel to different IC24 sites as required.
What We Expect:
- Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
- Advanced assessment and minor illness qualifications (Level 6 or 7).
- Independent prescribing qualification (Essential).
- Proven experience in autonomous practice and clinical leadership.
- Strong communication skills, able to manage complex and sensitive situations.
- Ability to prioritise, problem-solve, and make decisions under pressure.
- Commitment to continuous professional development and reflective practice.
Desirable:
- Experience in chronic disease management.
- Teaching and assessing qualification.
- Remote consultation experience.
- Evidence of service improvement and innovation.
