Are you an experienced healthcare professional ready to take the next step in your career? We are looking for a highly skilled Advanced Clinical Practitioner (ACP) to join our dynamic team, providing exceptional patient-centered care.
As an ACP, you will work autonomously at an advanced level, using high-level decision-making skills to assess, diagnose, and treat patients across a wide range of primary and urgent care needs. Your role will include:
- Telephone triage & remote assessments – Ensuring patients receive the right care, at the right time.
- Face-to-face clinical assessment & diagnosis – Delivering expert care for undifferentiated and urgent conditions.
- Management of all general practice needs – Including paediatrics, elderly care, and mental health.
- Independent prescribing – Working within current prescribing legislation to optimise patient treatment plans.
Who We’re Looking For:
- Registered healthcare professional (Nurse, Paramedic, or Allied Health Professional) with advanced clinical training.
- Proven experience in urgent or primary care settings.
- Strong decision-making skills and ability to work autonomously.
- A passion for delivering high-quality, patient-focused care.
