Care Co‑ordinator
IC24 seeks a compassionate, organised and proactive Care Co‑ordinator to support patients with long‑term conditions, complex needs and changing health circumstances. The role involves coordinating personalised care plans, acting as a central point of contact, and working closely with GPs, nurses, social prescribing link workers, and wider community services to deliver a seamless, patient‑centred experience.

Integrated Care 24 (IC24) is a leading not‑for‑profit social enterprise that provides innovative primary care services, including GP‑led out‑of‑hours, NHS 111, and secondary care support. We aim to improve access to health and social care, reduce demand on secondary services and ensure patients receive the right care at the right time.
Key Responsibilities
- Manage a caseload of patients, working closely with them, their families and carers to develop care and support plans that reflect individual needs and goals.
- Act as the central contact for patients, helping them navigate appointments, services and community support with a joined‑up, seamless experience.
- Empower patients to manage their health by providing information, guidance and practical support for self‑management programmes.
- Collaborate across multidisciplinary teams, including GPs, nurses, social prescribing workers, coaches and other health professionals to coordinate person‑centred care.
- Support multidisciplinary team meetings, contribute to palliative care and safeguarding processes, and assist with Quality and Outcomes Framework (QOF) activities.
- Identify opportunities to improve patient pathways, strengthen collaboration and enhance overall patient experience.
Qualifications & Experience
- Experience working in health, social care or another patient‑facing support role.
- Excellent communication and interpersonal skills.
- Highly organised with the ability to manage a varied workload and prioritise effectively.
- Experience collaborating within multidisciplinary teams.
- Proficient in Microsoft Office and quick to learn additional software.
- Compassionate, proactive and committed to delivering outstanding patient‑centred care.
- Passion for continuously learning, teamwork and service improvement.
