Integrated Care 24 (IC24) is recruiting an Advanced Clinical Practitioner for its Home Visiting Service. You will work autonomously in patients’ homes, using advanced assessment, examination, diagnostic and prescribing skills to deliver safe, patient-centred urgent care.
You’ll assess adults and children, balance clinical uncertainty, and decide on safe home-based care or escalation to other services, while staying within your professional scope and IC24 governance framework.
