Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner to join our Home Visiting Service. You will assess and treat patients in their homes, make autonomous clinical decisions and work with a wider IC24 team to ensure safe, compassionate care.
You will be an independent prescriber with advanced clinical skills, managing complex cases including end‑of‑life and frail patients in community settings. Part‑time hours up to 20 per week are available.
