JOB DESCRIPTION

Insight Global is looking for an In-Home Physical Therapist to service join a Home-Health Organization in the Greater Boston, MA area. This traveling PT would service clients at their homes located in hubs including- Concord, MA Marlborough, MA Hudson, MA Needham, MA Plymouth, MA & Taunton, MA. General

Responsibilities include:

1. Provides physical therapy services to patients according to a written physician’s plan of care period this may include but will not be limited to:

a. Assessing and evaluating therapeutic rehabilitative slash functional status and participation into the development of a total plan of care period evaluates home environment.

b. Assessing for muscle strength, mobility gate, ROM potential for rehab.

c. Directing physical therapy treatment.

d. Instructing patients and families/caregivers in the use of in care of therapeutic appliances

e. Determining priority needs physical therapy.

f. Reporting to position patients’ reaction to treatment or change in condition

2. Initiating physical therapy program and instructs other personal and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of their physical therapy program for the patient by participating in case conferences.

3. Train patient in use of prosthetic devices.

4. Identifies the patient and family/caregiver needs for other home health services and refers as needed.

5. Prepares and submits clinical progress summaries based on the attainment of goals.

7. Prepares and submits a clinical process summary based on the attainment of goals as directed by the organization policy.

8. Consult to home families/caregiver indicated.

9. Education programs for nursing organization personnel as needed.

10. Participates in peer consultation process.

11. Supervisors and physical therapy assistance according to organization policy and state regulations.

12. When therapy is the only skilled service, instructs, supervisors and evaluates home health aide care every two weeks.

Compensation:

$115,000 to $135,000 per year annual salary.
