Community IV Nurse in Coventry

As a Community IV Nurse , you will provide high-quality, patient-centred intravenous therapy and specialist nursing care to patients in their own homes and community settings.

You will support patients throughout their treatment journey, from therapy initiation and assessment through to ongoing management and completion of treatment. This will include delivering IV therapies safely, providing education to patients, families and carers, monitoring treatment response, and supporting patients to manage their care as independently as possible.

Working closely with pharmacists, consultants, specialist nurses and other healthcare professionals, you will play an important role in delivering safe and effective care outside of the hospital environment.

You will manage your own community caseload, plan and undertake patient visits, maintain accurate clinical records, and contribute to service quality through audit, governance and continuous improvement.

Key Responsibilities

• Deliver safe and effective IV therapies in patients' homes and community settings in accordance with Service Operating Procedures, clinical guidelines and prescribed treatment plans.
• Undertake patient assessments and provide ongoing monitoring throughout the course of treatment.
• Administer prescribed intravenous medications and therapies safely and effectively.
• Provide education, training and ongoing support to patients, families and carers.
• Support patients to develop confidence, independence and adherence to their prescribed treatment.
• Undertake home and community visits for therapy initiation, administration, assessment and follow-up.
• Provide appropriate care and management of vascular access devices in accordance with clinical policies and infection prevention standards.
• Recognise changes in a patient's condition and take appropriate action, including escalation to relevant healthcare professionals.
• Identify, document and escalation adverse events, incidents and clinical concerns appropriately.
• Maintain clinical equipment and ensure safe storage, handling and use of medicines and associated supplies.
• Maintain accurate, timely and comprehensive clinical documentation using relevant digital systems.
• Work collaboratively with multidisciplinary teams, including pharmacists, consultants, specialist nurses and other healthcare professionals.
• Manage and prioritise an allocated caseload effectively, including planning community visits and responding to changing clinical requirements.
• Adhere to safeguarding, infection prevention, medicines management, clinical governance and regulatory requirements.
• Contribute to clinical audit, service improvement and the ongoing development of community nursing services.
• Maintain professional registration and take responsibility for ongoing Continuing Professional Development (CPD).

What We're Looking For

Essential

• Registered Nurse with current NMC or NMBI registration .
• Minimum of 1 year post-registration nursing experience .
• Full UK or Irish driving licence.
• Strong clinical assessment, communication and interpersonal skills.
• Excellent organisational and time-management skills.
• Ability to work autonomously in a community environment and confidently manage an individual caseload.
• Ability to recognise clinical deterioration and and escalation concerns appropriately.
• Confident using digital systems for clinical documentation and communication.
• Enhanced DBS (or equivalent) and satisfactory medical clearance.

Desirable

• Previous experience delivering IV therapies, infusion services or community-based nursing care .
• Experience with vascular access devices and IV medication administration.
• Experience within areas such as oncology, gastroenterology, infectious diseases, nutrition support, homecare or other specialist therapies.
• Experience supporting patients receiving complex or long-term therapies in their own homes.
• Relevant degree, specialist nursing qualification or post-registration clinical qualification.
