We are seeking three experienced and motivated Senior Community IV Nurses to join our Patient Solutions team, delivering high-quality care as part of the Baxter Homecare Programme.
Main duties of the job:
- This is a community role where you will combine clinical expertise with leadership responsibility, supporting both patients and a small team of nurses across your region.

Job responsibilities:
- Lead, support, and mentor a small team of nurses (up to 5)
- Deliver safe, effective, and patient-centred care in homes, hospitals, and via virtual consultations
- Support patients transitioning from hospital to community care
- Build and maintain strong relationships with patients, carers, NHS hospitals, and key stakeholders
- Monitor and manage performance against KPIs, audits, and service standards
- Deputise for the Nurse Manager when required
- Participate in the on-call rota

Person Specification:
Experience:
- Current NMC registration
- Minimum 2 years post-registration experience
- Demonstrable leadership or mentoring experience
- Excellent communication and interpersonal skills
- A strong patient-focused approach
- Full UK driving licence and willingness to travel across your region
- Experience with CVAD, parenteral nutrition, or similar therapies
Qualifications:
- NMC Registered Nurse with 2+ years post-registration experience
