We are seeking an experienced and compassionate Clinical Nurse Advisor to support patients receiving Total Parenteral Nutrition (TPN) through a specialist outpatient homecare service. This role focuses on delivering high-quality clinical care, education, and ongoing support to patients living with complex nutritional needs, enabling them to safely and confidently manage TPN therapy in their own homes and community settings. - Clinical care & patient education
- Deliver high-quality clinical care in line with agreed Service Operating Procedures
- Provide structured education and ongoing support to patients, families, and carers receiving TPN therapy
- Support patients within their own homes and approved community settings
- Assist with therapy initiation, follow-up visits, and ongoing assessment
- Promote patient confidence, independence, and adherence to therapy
- Maintain and manage all clinical equipment and materials
- Collaboration & service delivery
- Build strong working relationships with dietitians, nutrition nurses, pharmacists, and wider healthcare teams
- Support outpatient pathways and continuity of care between hospital and home
- Deliver services in line with agreed KPIs and service expectations
- Identify, manage, and escalate adverse events in line with policy
- Complete all clinical documentation, activity reporting, and administration accurately and on time
