The predicted salary is between 37338 - 44500 Β£ per year.

We are seeking three experienced and motivated Senior Community IV Nurses to join our Patient Solutions team, delivering high-quality care as part of the Baxter Homecare Programme.

Main duties of the job:

  • This is a community role where you will combine clinical expertise with leadership responsibility, supporting both patients and a small team of nurses across your region.

Job responsibilities:

  • Lead, support, and mentor a small team of nurses (up to 5)
  • Deliver safe, effective, and patient-centred care in homes, hospitals, and via virtual consultations
  • Support patients transitioning from hospital to community care
  • Build and maintain strong relationships with patients, carers, NHS hospitals, and key stakeholders
  • Monitor and manage performance against KPIs, audits, and service standards
  • Deputise for the Nurse Manager when required
  • Participate in the on-call rota

Person Specification:

Experience:

  • Current NMC registration
  • Minimum 2 years post-registration experience
  • Demonstrable leadership or mentoring experience
  • Excellent communication and interpersonal skills
  • A strong patient-focused approach
  • Full UK driving licence and willingness to travel across your region
  • Experience with CVAD, parenteral nutrition, or similar therapies

Qualifications:

  • NMC Registered Nurse with 2+ years post-registration experience
