Overview
In this role you will lead the COTE / Frailty service within an NHS district general hospital. You will deliver Comprehensive Geriatric Assessment (CGA) and manage complex frail and multimorbid older adults. You will work closely with Emergency Department, Acute Medicine, Therapies and Community teams to optimize patient flow and care quality. The post offers leadership in governance, audit, teaching, and service development within a supportive, collaborative environment. This is an opportunity to shape frailty pathways and deliver patient-centred care at scale.
Responsibilities
- Deliver Comprehensive Geriatric Assessment (CGA)
- Manage complex older adults with frailty and multimorbidity
- Collaborate with Emergency Department, Acute Medicine, Therapies, and Community teams
- Participate in General Internal Medicine on-call rota
- Engage in governance, audit, teaching, and service development
Key requirements
- GMC Registration with Licence to Practise
- CCT in Geriatric Medicine / General Internal Medicine (or equivalent)
- Experience within Acute Frailty / COTE services
- Commitment to MDT working and quality improvement
