Support Worker – Care Leavers

Location: Darwen/Nelson

Contract: Full-time

Hours: 24hrs including sleep in\’s (rota)

About the Role

We are seeking a dedicated Support Worker to provide high-quality support to young people aged 16–25 who are transitioning out of care. You will help care leavers develop independence, build practical life skills, and work towards stable, positive futures.

Key Responsibilities

  • Provide emotional, practical, and social support to young people leaving care.
  • Support with budgeting, cooking, tenancy management, and other life skills.
  • Develop, implement, and review personalised support plans.
  • Promote independence while offering guidance and stability.
  • Work confidently and safely during lone working shifts, following all policies and safeguarding procedures.
  • Communicate effectively with multi-agency partners, including social workers, local authorities, housing teams, education providers, and health professionals.
  • Maintain clear, accurate, and timely records.
  • Support access to education, training, and employment opportunities.
  • Encourage young people to build resilience, confidence, and positive routines.

About You

You will be someone who is:

  • Passionate about supporting vulnerable young people.
  • Empathetic, patient, and able to build strong, trusting relationships.
  • Organised, reliable, and calm under pressure.
  • Comfortable working both independently (including lone working) and as part of a team.
  • Able to work flexibly, including some evenings and weekends.

Essential Requirements

  • Level 3 Diploma in Health & Social Care (or equivalent qualification).
  • Experience of lone working and the ability to manage risk and make confident decisions independently.
  • Experience supporting young people or vulnerable individuals.
  • Strong communication skills with experience liaising with multi-agency professionals.
  • Understanding of safeguarding, professional boundaries, and trauma-informed practice.
  • Good record-keeping and report-writing skills.
  • Enhanced DBS check (or willingness to complete one).
  • Driving licence (desirable but not essential).
