We are seeking a dedicated ICON Co-occurring to provide a comprehensive Tier 3/4 service to people experiencing substance use problems. ICON aims to bridge the gap between substance misuse and mental health services, recognising that for our cohort of service users, co-occurring needs are the norm, not the exception. Through this project, we aim to reduce barriers to access treatment, duplication, and fragmentation.
Main duties of the job:
- Provision of drug & alcohol treatment to Service Users within the locality.
- Holding a caseload of service users and being responsible for offering a range of interventions to those individuals.
- Supporting service users through their treatment journey from beginning to end.
- Embracing the concept of recovery and demonstrating this in your clinical work.
- Ensuring that your work is evidence-based, up to date and in accordance with current policy and legislation.
- Keeping up to date and accurate records and documentation.
Job responsibilities:
- Provide harm reduction advice, information and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A, B and C and other blood borne infections.
- Offer Naloxone to all Clients due to risks presenting, or as a precautionary measure.
- Be responsible for maintaining a full caseload of complex clients as the key worker.
- Conduct outreach visits consistent with need.
- Offer a flexible approach to work within a range of locations including STARs hubs, MPFT hubs and an outreach setting.
- Carry out comprehensive assessments, construct care packages with multi-disciplinary colleagues where required, review care plans in a structured format and manage onward referral.
- Assist and support medical and nurse prescribers during harm reduction and prescribing clinics.
Person Specification:
- Qualifications: Foundation Degree / HNC in related field which may include counselling, social work, nursing, psychology, human resources, social studies or equivalent experience.
- Experience: Considerable experience of working with drug users. A commitment to and understanding of harm reduction & recovery approaches to working with drug users. Counselling skills, case management experience & the ability to work under pressure. A sound experience of working with techniques such as CBT, Motivational interviewing and Solution focused therapy.
- Knowledge, Skills and Abilities: A thorough knowledge of substances and their effects. Ability to work with drug/alcohol users in a community environment. Ability to work under pressure. The ability to work flexibly across the county and adapt to meet the needs of the service.
