Inclusion Healthcare CIC in Leicester is seeking a Homeless Healthcare Co-ordinator for a 6-month fixed-term pilot. You will work across healthcare, housing, social care and voluntary sectors to improve access to care for people experiencing homelessness and multiple disadvantage.
You will build relationships with patients and partner agencies, coordinate care, support nurse-led clinics, and help reduce barriers to healthcare through trauma-informed, person-centred approaches.
