Inclusion Healthcare has an exciting opportunity for an Advanced Nurse Practitioner to join our friendly and dedicated practice team, providing care to the homeless and asylum seeker population in Leicester and Leicestershire. We are a not-for-profit social enterprise delivering responsive, compassionate primary care to groups at risk of marginalisation.

With over 25 years experience supporting the homeless community and more than 10 years working with asylum seekers, we operate three GP practices and the No.5 Recovery Hub – a day centre for people with street lifestyles who use alcohol or substances. This is a rare and rewarding opportunity to make a real difference in the lives of patients with complex needs. We’re looking for someone with clinical expertise, empathy, and a genuine commitment to improving access to healthcare for marginalised populations.

Main duties of the job

To provide advanced, autonomous, and patient-centred care for patients registered with Inclusion Healthcare, with a particular focus on people at risk of marginalisation, including those who are homeless, seeking asylum, and patients with complex social needs. The ANP will assess, diagnose, and manage undifferentiated medical conditions, prescribe medications within competence (including controlled medicines where appropriate), and develop holistic care plans. They will work collaboratively within a multi-disciplinary team to ensure integrated care pathways and improve health outcomes for vulnerable populations.

The post holder will demonstrate advanced clinical assessment and decision-making skills, working autonomously within their scope of practice.
