About The Role
As a Care Planner you'll work closely with people who use our services, their families and our commissioning partners to develop support plans that are genuinely person-centred, measurable and compliant. This is a contract role suited to an experienced professional who wants to contribute to a new service model from the ground up.
Contract: Stoke-on-Trent, Staffordshire
Responsibilities:
- Carry out assessments and develop detailed, person-centred support plans that genuinely reflect the individual's goals, routines and preferences.
- Work with the person, their family, social workers and commissioners throughout the planning process.
- Ensure plans meet relevant care standards and capture meaningful, measurable outcomes.
- Review and update support plans at regular intervals, responding to changes in need.
- Maintain accurate and compliant documentation in line with regulatory requirements.
- Support the organisation to develop consistent, high-quality planning processes.
What we're looking for:
- Demonstrable experience in care planning, case management or a related assessment role.
- Strong working knowledge of person-centred planning approaches and relevant legislation.
- Experience working with local authority commissioners and multi-agency teams.
- Excellent written skills — the ability to write plans that are clear, person-centred and professionally presented.
- NVQ/QCF Level 4 or above, or equivalent professional qualification (desirable).
- Enhanced DBS check required.
