South Southwark Primary Care Network Senior Clinical Pharmacist

Weare seeking a highly motivated and experienced senior clinical pharmacist tojoin our well-established and successful team of thirteen pharmacists and threepharmacy technicians. You will work across our network, providing dedicatedsupport to general practice and our local population. The post holder will workwith the Lead Pharmacist to provide support, direction and supervision to theappointed clinical pharmacists working across general practice and clinicalleadership on medicines management and quality improvement initiatives.

Thesuccessful candidate will be supported through shadowing, robust clinicalsupervision arrangements and work alongside other clinicians as part of amulti-disciplinary team in patient and non- patient facing roles. The postholder will be supported with ongoing clinical learning and professionaldevelopment.

Main duties of the job

This is an exciting role which will contribute to the reshapingof neighbourhood care for our populations, directed and supported by the SouthSouthwark Primary Care Network (PCN). The post holder will work within theirclinical competencies as part of a multi-disciplinary team to

• Provide direct line management of appointed clinicalpharmacist(s)
• Monthly one-to one meeting with appointed clinical pharmacist(s)
• Planning and implementation of new staff inductions withEducation and Training Lead
• Provide supervision as Designated Practitioner(DP) andDesignated Prescribing Practitioner (DPP) for foundation trainee pharmacist(s)
• Working with Education and Training Lead to deliver training
• support long-term condition management within the practiceincluding medication reviews.
• provide clinical leadership on medicines optimisation andquality improvement and manage some aspects of the quality and outcomesframework and enhanced services.
• Provide support on prescription/medication queries andprocesses.
• Facilitate improved working with community and hospitalpharmacy.
• Other general tasks as required.

You willcontribute to ongoing service developments, providing data and progressupdates, and sharing insights into how the PCN could better support and carefor its population.

About us

Our South Southwark PCN is made up of eighteen GP practices in fourneighbourhoods - Camberwell, Peckham, Nunhead and Dulwich, servingapproximately 157,000 patients, supported by their GP Federation, ImprovingHealth (IHL).

The NHS Long Term Plandescribes the prominent role Primary Care Networks play in deliveringproactive, personalised and more integrated health and social care for theirlocal populations. This will require collaborative working betweenorganisations including GP practices, acute and community health, social careorganisations and the voluntary and community sector. We are a large PCN with over 60 staff in post to date andexpanding. We are committed to supporting and developing our staff, knowingthis is crucial to our success. We haverobust systems in place, including clinical supervision and peer support, andwill continue to build on this as we expand our PCN team.

Job responsibilities

Thepost holder has a responsibility to adhere to all IHL policies, procedures andrelevant legislation including the requirements of any professional bodies;maintain satisfactory personal performances and professional standards and toachieve agreed objectives for their role; attend mandatory training asidentified by IHL and contribute to their own development.

Person Specification

Qualifications

• Mandatory registration with General Pharmaceutical Council (GPhC).
• Previous GP experience (Minimum 3 years)
• Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
• CPPE Primary Care Pharmacy Education Pathway

Experience

• Minimum of 5 years post qualification experience
• In-depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
• An understanding of general practice and challenges they currently face.
• An understanding of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
• Excellent interpersonal, influencing and negotiating skills.
• Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)
• Able to obtain and analyse complex technical information.
• Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
• Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
