Job overview
We are looking to recruit an experienced, motivated individual to join our complex discharge team. You will be predominantly based at Charing Cross Hospital site but will be expected to work across site as required.
We are looking for individuals with strong communication and interpersonal skills – highly motivated, with initiative and receptive to feedback. You will have critical thinking skills and be able to work well as part of a team.
The post holder will:
- Promote and work within the Discharge to Assess Pathway working collaboratively with patients and their families
- Work collaboratively with the ward teams to identify and assess patients care needs and co-ordinate services to support the safe and timely discharge of patients with complex needs, liaising with the multi-disciplinary team and partner agencies across acute and community settings
- Hold a case load of complex patients
- Be expected to actively participate in the education of your peers and other professional groups within the Trust
You will become part of a team with a wealth of experience, both clinically and administratively, who will be able to offer you support, training and guidance as you develop your career with us.
The post holder will be required to participate in the weekend and bank holiday service cover arrangements.
Main duties of the job
- To deliver high standards of patient care and pathway management to reduce delayed transfers of care across the trust, ensuring a safe, patient centred discharge.
- To undertake / coordinate the assessment of patients’ needs.
- To act as the SMH Discharge Hub screener on a rotational basis.
- To deputise for the Discharge Team Manager as required, attending Trust meetings where relevant to the role.
- To assist in identifying educational needs of staff in other services and contribute to developing and delivering educational opportunities to staff within the hospital and externally.
- To line manage, provide clinical supervision and support and undertake PDR’s with allocated junior staff.
- To work within the Trust’s Discharge Lounge when required ensuring patients are transferred home safely and to maximum overall utilisation of the unit.
- The post holder is required to work on the weekend and bank holiday rota as part of their contracted hours.

- Registered Nurse
- Teaching qualification or equivalent experience in hospital / community team setting
