Derbyshire County Council is seeking an experienced Adult Social Worker to join their hospital-based team in Chesterfield. This role is ideal for a practitioner with hospital experience who understands the Discharge to Assess (D2A) process and can manage complex adult cases effectively.
Key Responsibilities
- Fast-Paced Hospital Assessment: Conducting assessments to support safe and timely hospital discharges.
- Complex Casework: Managing a caseload of adults with complex needs across health and social care settings.
- Discharge Planning: Coordinating the Discharge to Assess process in collaboration with hospital and community teams.
- MCA & Safeguarding: Completing Mental Capacity Act assessments, leading safeguarding enquiries, and implementing risk management plans.
- Care Planning: Developing person-centred support plans and arranging appropriate services for individuals returning to the community.
Working Pattern
- Office Presence: Full-time on-site Monday to Friday, 9:00am - 5:00pm.
Requirements
- Registered Social Worker (Social Work England)
- Substantial hospital-based social work experience
- Strong understanding of the Discharge to Assess model
- Experience with complex case management, MCA, and safeguarding work
- Excellent communication and multi-agency working skills
