Referral CoordinatorPermanent£30-34kBirminghamHamilton Woods Associates are currently recruiting for a Referral Coordinator / Community Liaison Officer to join an established supported accommodation provider. The successful candidate will develop referral pathways, manage incoming referrals and build relationships with organisations supporting vulnerable adults.Duties and Responsibilities:- Develop referral pathways with Local Authorities, NHS services, Probation, Social Workers, Homelessness teams and charities- Build and maintain strong relationships with referral partners- Promote available accommodation and services to relevant professionals- Manage referrals from enquiry through assessment, viewing and placement- Attend stakeholder and networking meetings- Work closely with Housing and Support teams to ensure appropriate placements- Monitor referrals, occupancy and voidsEssential Requirements:- Experience within supported accommodation, housing, homelessness, social care, referrals or community engagement- Experience managing referrals, placements or assessments- Strong relationship-building and networking skills- Excellent communication and organisational skills- Experience working with Local Authorities, NHS services, Housing Providers or other professional agencies- Ability to work independently and manage multiple priorities- Full UK Driving Licence and access to a vehicle- Applicants must already have the legal right to work in the UK.
