We are seeking Support Workers to join our team and provide exceptional care and support to young people and adults with complex needs in the community. As a Support Worker, you will play a crucial role in promoting independence, supporting with care plan interventions and enhancing the quality of life for our clients.
What are you responsible for:
- Provide one-on-one support to individuals with complex needs.
- Assist clients with daily living activities, personal care, and medication management.
- Foster a safe and supportive environment, promoting physical and emotional well-being.
- Collaborate with a multidisciplinary team to ensure care plans are being adhered to.
- Support clients in building life skills and participating in daily social/community activities.
- Document client progress, communicate observations to the Clinical Lead or Registered Manager.
- Provide emotional support, managing challenging behaviours and ensure clients' voices are heard.
Required skills:
- Compassionate and empathetic with a strong desire to support others.
- Previous experience in a support worker role is advantageous, minimum 3 years.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- A commitment to promoting independence and enhancing the lives of individuals with complex needs.
- Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
- NVQ Level 3 in Health and social care.
To be considered for this position, you will demonstrate exposure to the following areas of expertise: mental health, complex health needs, physical health needs, hospital discharge to community rehabilitation, autism and learning disabilities, life-limiting conditions, working with children and young people / adults aged 18 – 65 and 65 and above.
