Imago Community in Southwark is seeking a full-time practitioner to support Young Carers across the borough. You will manage an existing caseload and new referrals, meeting with families and Young Carers to complete statutory assessments and create action plans; driving is preferred.
You will provide support to reduce the impact of caring responsibilities, improve resilience, and signpost to local services, in-school support, and one-to-one sessions.
