CHC Assessor Requirements:
- Valid NMC pin
- A minimum of 12 months post-qualification experience
- A minimum of 6 months' experience as CHC Assessor
- Appropriate workspace within home setting that allows you to successfully complete your role, including broadband connection (all equipment provided for the role)

CHC Assessor role:
- Annual salary of £38,000 - £44,962 (depending on experience)
- Induction and training at the beginning of employment
- Chair Multi-Disciplinary Meetings (MDTs)
- Conduct reviews and assessments on behalf of Integrated Care Boards (ICBs) and Health Boards
- Assessments are carried out virtually via MS Teams/Zoom with the individual, their provider, and family (or representatives)
- Deal with highly sensitive, highly complex information in relation to changes to service provision
- Produce reports that can be understood by individuals, their representatives and clinical audiences
- Liaise with a range of clinical peers and services across the NHS and private healthcare sector to obtain evidence that assists in the production of detailed reports
- Plan and organise effectively to achieve group and personal targets
