Help build the clinical capacity that enables people with highly complex needs to leave hospital and live safely in the community.

Location: South West England, ideally within reach of Bristol

Working pattern: Full time or part time considered

Contract: Permanent or Part Time

This is a rare opportunity to join a specialist community healthcare provider rated Good by CQC, operating as part of a wider established care group with services that have achieved Outstanding CQC ratings. The provider works with people whose needs are often considered too complex for conventional community services. This includes individuals with combinations of physical health needs, mental health needs, neurological conditions, learning disabilities, autism and behaviours that may challenge.

Its teams mobilise high intensity packages, prevent avoidable hospital admissions and support people through complex transitions from hospital into their own homes. This is not a traditional shift based nursing role and it is not a position where you will simply inherit a static caseload. You will help build the clinical capacity that allows the service to accept more complex physical health packages while protecting the quality, safety and individualised approach that underpin its current CQC rating.

The organisation has already established a strong specialist community model and multidisciplinary team. Its next stage of growth requires additional general nursing expertise. You will clinically assess new packages, prepare care teams, oversee delivery and make sure each person receives safe and appropriately governed support. For the right person, there is genuine scope to influence how the physical health provision develops.
